Statistics notes: Concealing treatment allocation in randomised trials.

چکیده
why depending on the rationality of the market will never work well for quality of care (box). Sensible policy for providing nursing home care requires a larger welfare state, a larger regulatory state, and encouragement of public, non-profit providers. Australia’s recent experience shows that to head in the opposite direction is medically, economically, and politically irrational. Competing interests: None declared.

Statistics notes. Treatment allocation in controlled trials: why randomise?
Since 1991 the BMJ has had a policy of not publishing trials that have not been properly randomised, except in rare cases where this can be justified. Why? The simplest approach to evaluating a new treatment is to compare a single group of patients given the new treatment with a group previously treated with an alternative treatment. Usually such studies compare two consecutive series of patien...
متن کاملStatistics notes: missing outcomes in randomised trials.
In most randomised trials, some patients fail to provide data for study endpoints. 1 We have previously described the analysis of a trial of acupuncture versus sham acupuncture for the treatment of shoulder pain. 2 All 52 randomised patients provided baseline data on pain and range of motion, but only 45 returned for follow-up testing. The statistical question is how to handle those seven patie...

OBJECTIVE To summarise comparisons of randomised clinical trials and non-randomised clinical trials, trials with adequately concealed random allocation versus inadequately concealed random allocation, and high quality trials versus low quality trials where the effect of randomisation could not be separated from the effects of other methodological manoeuvres. DESIGN Systematic review. SELECT...
متن کاملBlindness in Randomized Controlled Trials
In combination with randomization, blinding or masking is an important factor in randomized controlled trials (RCTs), particularly in trials that assess therapeutic effects. Here an attempt is made to explain blindness and why it is important. In clinical trials, blinding is defined as the condition imposed on a study in which study participants, health care providers and assessors collecting o...
متن کاملMechanisms and direction of allocation bias in randomised clinical trials
BACKGROUND Selective allocation of patients into the compared groups of a randomised trial may cause allocation bias, but the mechanisms behind the bias and its directionality are incompletely understood. We therefore analysed the mechanisms and directionality of allocation bias in randomised clinical trials. METHODS Two systematic reviews and a theoretical analysis. We conducted one systemat...
